Simultaneous Detection of Cold Agglutinin and anti-M RBC Antibody in a patient with Hemolytic Anemia

Abstract
The cold agglutinin syndrom is charaterized by abnormal titers of cold agglutinins and
the clinical manifestations caused by cold agglutination and by complement-dependent
hemolysis, and complement mediated enhancement of phagocytosis of non-hemolyzed red
cells. The authors here presented a case of hemolytic anemia associated with cold
agglutinin and simultaneous detection of anti-M RBC antibody in a 36 year male Patient
who was admitted to Dept. of chest surgery for treatment of known cavity due to
pulmonary tuberculosis on Sept. 4, 1980. On admission, physical examination revealed
normal except decreased breathing sound. On the 4th hospital day, explothoracotomy and
right upper lobectomy were done and four units of whole blood were transfused. On the
32nd post-operative day, sudden development of jaundice with anemia and dark colored
urine were noted, so that the patient was transferred to Dept. of Int. Med. for further
examination and treatment. Bone marrow smear showed increased cellularity with
erythroid hyperplasia. During the cross-matching Procedure for transfusion, the presence
of cold agglutinin was found and it's titer was elevated to 1 : 256. Unexpectedly
simultaneous existence of anti-M RBC antibody was identified in the serum. He was
treated with transfusion and corticosteroid until discharge with marked improvement.
